Attached you will find the application forms for Community Christmas Closet. The Community Christmas Closet provides clothing to children of low-income families in Midland County. The target population is grades K-12. They also provide clothing to children who attend pre-school programs who wear size 4 or larger. Each child receives a new outfit including jeans, a sweatshirt, two pair of underwear, two pair of socks, a hat or headband and mittens or gloves.
Applications will be accepted at the Department of Human Services from now until the deadline of Friday, October 28, 2016. You can also mail the completed forms to: The Community Christmas Closet, P.O. Box 2255, Midland, MI 48641. Please be sure that the applications are returned on or before the deadline.
If you need a printed copy, please respond to this email and I will send a form home with your child.
Please remember – completed application forms do NOT come back to school – the forms need to go to the Department of Health Services or mailed to the address listed above.
